# Failure Intelligence Engine (FIE)

**A 23M-parameter offline LLM guardrail — and an honest measurement of why guardrails fail.**

## Three findings you can check yourself

Most of this project's value is not the detector. It is what fell out of trying
to evaluate the detector honestly.

### 1. The benchmarks everyone uses are contaminated

We audited our own training data against the standard attack benchmarks and
found **52.5% of JailbreakBench and 67.5% of AdvBench had leaked into it.**

We retrained on decontaminated splits, quarantined AdvBench as a contamination
case study, and republished **lower** numbers. If you train a guardrail on these
benchmarks without an audit, your reported recall is partly memorisation.

### 2. Over-refusal is far worse than anyone reports — including us

Our previously-reported 9% false-positive rate was measured on *naturally
occurring* benign prompts. On the field's **standardized over-refusal
benchmarks** — prompts engineered to look harmful but be completely safe:

**Our own 9% number understated this by roughly 8×.** 47% of these false
positives are high-confidence (mean 0.76), so no threshold tweak fixes it — the
sweep has **no operating point with both low over-refusal and high recall**.

### 3. This is not a small-model problem — it defeats a 20B guard too

Measured against **gpt-oss-safeguard-20b**, a 20-billion-parameter policy guard:

- The big model out-detects FIE (94% macro recall) and reaches an XSTest
  operating point FIE cannot. **FIE's value is deployability, not detection
  supremacy.**
- But **OR-Bench-hard defeats the 20B guard too, at 80% over-refusal.**

The over-blocking blind spot is not a capacity problem. It is field-wide, and
almost nobody measures it.

## The tool

If you want a guardrail rather than the research, here it is.

No API key, no network, no configuration. ~22 ms per scan, offline, with
bundled models.

**Warm up before serving traffic** — model loading is lazy, so the first scan
otherwise pays ~10 s and runs with reduced coverage:

## What it detects, and what it does not

**Detects** (offline, in parallel, ~22 ms): prompt injection · jailbreaks (DAN,
persona, override tags) · token smuggling · many-shot conditioning · encoded
payloads (Base64, leetspeak, homoglyphs) · indirect injection in documents ·
GCG adversarial suffixes · fiction/roleplay wrapping · multilingual injection ·
crescendo multi-turn escalation · copyright reproduction.

**Does not reliably detect:** hate speech, harassment, or self-harm phrased as
ordinary conversation without adversarial structure. FIE is a failure-diagnosis
system, not a general content moderator. The numbers below reflect that scope.

### Attack recall (decontaminated, held-out)

Macro **85.8%** across four leakage-audited benchmarks, versus ~22–26% for
offline injection/jailbreak baselines.

Per-method on JailbreakBench: JBC 100%, PAIR 100%, **GCG 73.7%** — adversarial
suffixes are the genuine weak spot.

### Out-of-distribution false positives

PAIR v5's reported 4% FPR was measured on its own training distribution.
Domain-balanced retraining fixed it — **no architecture change, only the training
data changed**:

### What actually carries detection

A full layer ablation on leakage-free data shows **the semantic classifier (PAIR)
carries the common case** — on standard benchmarks it *alone* matches the full
pipeline, and removing any other single layer changes recall by ~0.

Specialized layers do earn their place on their own vectors: `multilingual`
(25% → 100% on non-English) and `copyright` (12.5% → 100% on verbatim
reproduction). But the twelve-layer count is not what makes this work, and we
say so.

## Two methodology findings

- **In-distribution metrics lie.** Validating a guardrail on benign data drawn
  from its own training distribution dramatically understates real-world FPR.
- **"Forbidden" ≠ "adversarial."** Several popular "harmful" datasets mix in
  benign or liability-restricted-but-harmless requests (financial, legal, health
  advice). Training on those as attacks re-introduces false positives.

## Architecture

Every prompt is screened **before** it reaches your model; every answer is
checked **after**, in the background.

## Usage

### Scan a prompt directly

`degraded_layers` matters. A non-empty list means a layer timed out or errored,
so this verdict was produced with **reduced coverage** — `is_attack=False` is
weaker evidence of safety than usual.

### Explain a decision

Shows every layer's score, whether it fired, and its evidence. The fastest way to
debug a false positive.

**Requirements:** Python 3.9+, MongoDB Atlas (free tier), Groq API key (free,
for hallucination monitoring only).

## Known limitations

Stated plainly, because a guardrail whose limits you don't know is worse than none.

- **Over-refusal** — 53.6% XSTest / 90.4% OR-Bench-hard on safe prompts. The
  headline finding above, and the project's biggest open problem.
- **GCG suffixes** — 73.7% recall, the genuine weak vector.
- **Euphemism / paraphrase** — harm described without trigger words evades
  detection roughly half the time.
- **Content moderation** — not the design target. See scope note above.
- **White-box evasion** — anyone who reads this codebase can craft a prompt below
  threshold. FIE is not designed to be a black box.
- **`torch` is 1.32 GB** — currently blocks free-tier hosting. Migration path
  documented in [docs/DEPLOYMENT.md](docs/DEPLOYMENT.md).

## Contributing

Built and maintained solo — contributions genuinely welcome, and the codebase was
recently restructured to make them possible.

Good places to start:

- **Add a detection layer** — each lives in its own module under
  [`fie/layers/`](fie/layers/); see the walkthrough in
  [CONTRIBUTING.md](CONTRIBUTING.md).
- **Break the detector** — the euphemism/paraphrase gap and GCG recall are known
  weaknesses. Send prompts that get through, via the Detection Report issue
  template.
- **Report a false positive** — over-refusal is the biggest open problem. Real
  benign prompts that get blocked are directly useful.

Before changing anything in the request path, read
[docs/PRODUCTION_ENGINEERING.md](docs/PRODUCTION_ENGINEERING.md) and run:

The golden test locks detection output to 4 decimal places. If your change moves
a number, that is either a bug or a result — and either way it needs to be
explained, not merged silently.
