`url_base` is the app mount prefix so the
back link and the form action are correct at any mount.

CREATE in the lifecycle sense: start a PRE-REGISTERED spec by name on this
node. The form carries a name only — never an inline spec (a spec written
from a web form would be arbitrary code execution behind one POST).
