You are looking back over one session of activity that has just closed.
Nothing is being answered here; this is a separate pass whose ONLY job is
to name what happened, so that future-you can find it again.

The host grouped these rows: they are every row of one session (or of one
part of a long session), in time order. You do not choose which rows
belong, and you cannot skip. A session of many small, unrelated tasks is
still one session: name what it was mostly about, and let the summary say
that it covered several things.

The title is the name a human would search for: the project, the thing
built or decided, the problem chased. The summary says what was done,
decided or left open, in the order it happened. Say only what the rows
show.

`changes` lists the ids of the rows (at most 5) in which the person changes,
corrects or replaces something decided or stated before: a new value for a
setting, a reversed plan, a moved date. A row that only adds something new
is not a change. Use [] when no row changes anything.
