EXAMINATION: CHEST (PORTABLE AP)

INDICATION: ___ year old man with pancreatitis, s/p dobhoff advancement // ? dobhoff placement post pyloric ? dobhoff placement post pyloric

IMPRESSION:

Transesophageal feeding and drainage tubes and in the distal and proximal stomach respectively. Indwelling ET tube and left subclavian central venous line are in standard placements respectively. No pneumothorax. Although heart size is normal mediastinal widening indicates increased persistent increase in central venous volume or pressure. Compared to chest radiographs ___ and ___. Moderate to severe pulmonary edema and moderate bilateral pleural effusions developed over preceding 24 hr have both improved, but lung volumes remain very low, reflected in severe left lower lobe atelectasis. The Small region of subpleural consolidation at the left apex which may have started as a smaller lesion on ___ should be followed. It could be pneumonia or infarction.
