 progressive dyspnea/cough; found to have multifocal PNA; now c/b hypoxic

 IMPRESSION:

 No pneumothorax.
 ET tube and right internal jugular line are in standard placements.
 Transvenous right atrial right ventricular pacer leads are unchanged in their positions.
 Esophageal drainage tube passes into the stomach and out of view.
 Small bilateral pleural effusions are unchanged.
 Heart size has decreased and pulmonary and mediastinal vascular engorgement have improved.
 Extensive pulmonary consolidation and concurrent mild pulmonary edema have improved since ___.
 Although there is stimulation of cavities by virtue of emphysema, there is also cavitation due to necrotizing pneumonia and/or septic emboli.
