Sign in with your Apple Account in Xcode
Xcode is opening. Apple only allows adding an account inside Xcode itself (it may ask for a 2-factor code) — one time only.
In Xcode: menu Xcode → Settings… (or press ⌘ ,)
Open the Accounts tab, click + (bottom-left), choose Apple Account
Sign in with your Apple ID — then come back here
watching for the new account — this page updates by itself

One-time: trust the developer on your iPhone
The app is already installed. iOS needs you to trust the developer once — Apple only allows this by hand on the phone, it can't be automated. After the first time it's remembered, so future runs skip this.
On the iPhone: Settings → General → VPN & Device Management
Under Developer App, tap your Apple Development certificate
