Add-as-OOB action form, shared by the serial-match and primary_ip-match panes of
device_validation_details.html so the two can't drift (same rationale as
_existing_librenms_link_status.html). Expects the panes' rendering context:
validation (existing_device), libre_device (device_id) and server_key.
