Shared Agents create content. Included by create.html, which extends the SDK
adapter (scitex_sdk/app/app_shell.html) and fills scitex_app_content.
`url_base` is the app mount prefix so the
back link and the form action are correct at any mount.

CREATE in the authoring sense: scaffold a fresh v3 spec.yaml + to_home/
skeleton through the SAME backend as `sac agents create`. The template
select is live-scanned from the agents root on every GET (inline
`minimal`/`full` plus any `_template_*` dir-templates), so the list cannot
go stale. The GUI never overwrites: an existing spec fails loud (409) and
the operator re-runs with `sac agents create --force` on the leaf.
