Employee Benefits Overview

The company offers comprehensive benefits including medical, dental, and
vision insurance through BlueCross BlueShield. Three medical plan tiers are
available: Basic, Standard, and Premium, with monthly employee contributions
ranging from $85 to $275 depending on coverage level and family status.

Open enrollment occurs annually from October 15 through November 15. New
employees must complete benefits enrollment within 30 days of their start
date. Qualifying life events such as marriage, birth of a child, or loss
of other coverage allow mid-year enrollment changes within 30 days of the
event.

Retirement benefits include a 401(k) plan with company matching up to 4%
of salary. The company match vests on a graduated schedule: 25% after one
year, 50% after two years, 75% after three years, and 100% after four years
of service. Employees may contribute up to the IRS annual limit.

Additional benefits include a Health Savings Account with a $500 annual
company seed contribution for employees on high-deductible plans, an Employee
Assistance Program providing confidential counseling and referral services,
tuition reimbursement up to $5,250 per calendar year for approved degree
programs, and commuter benefits with pre-tax transit and parking deductions.

Life insurance at 1x annual salary is provided at no cost. Supplemental
life insurance is available at group rates up to 5x annual salary with
evidence of insurability required above 3x.
