# Failure Intelligence Engine (FIE)

**Adversarial detection + hallucination monitoring for LLMs — blocks prompt injection, jailbreaks, and adversarial inputs before they reach your model. Monitors outputs for factual failures and hallucinations. Built for developers who want to understand _why_ an interaction is likely to fail, not just that it did.**

FIE wraps any LLM with a single decorator. It runs 11 detection layers in parallel on every incoming prompt, blocks confirmed attacks before the model runs, monitors outputs for hallucinations using a shadow ensemble and XGBoost classifier, and logs everything to a real-time dashboard.

## What's new in v1.12.0

### PAIR v4 — 97.18% TPR on novel held-out attacks

PAIR v4 uses 3× hard-positive weighting (Experiment 8 finding: 3× strictly outperforms 5× at the same FPR with higher F1 and better natural calibration). Evaluated on UnknownBench-v3 — a completely fresh held-out set that was never used in training.

The threshold drop from 0.80 to 0.50 is the key result. 5× weighting forces you to override the calibration manually. 3× achieves the same performance at the classifier's natural confidence boundary — a more honest model.

### FSV ablation — 10 features explain all signal

A SHAP analysis of the XGBoost failure classifier over the 560-feature Failure Signal Vector found that 10 features account for 100% of predictive performance (F1 = 0.8963 at top-10 vs 0.8960 full). The remaining 550 features contribute noise, not signal.

This mirrors the adversarial finding (architecture ≠ generalisation). In the failure classifier, more features ≠ better signal. Future versions will trim the FSV to this minimal sufficient set.

### TruthfulQA hallucination evaluation harness

A complete evaluation harness for measuring the XGBoost failure classifier on TruthfulQA (817 questions, 38 categories). Compares two approaches:

- **Exp H1** — Full FSV + XGBoost (10-feature offline threshold sweep)
- **Exp H2** — Ensemble disagreement alone (shadow model disagreement as a classifier)

### Hard-positive collection pipeline

UNCERTAIN-zone blocks (prompts that entered the [0.60×T, T) zone and were conservatively blocked) are now:

1. Recorded in the feedback review queue (they were invisible before)
2. Staged with full prompt text in a local file (when `FIE_COLLECT_HARD_POSITIVES=1`)
3. Promoted to confirmed hard positives via `POST /flags/{id}/label` → `true_positive`
4. Exported for the next PAIR retraining via `GET /flags/hard-positives/export`

This closes the most important training data gap: the prompts that are hardest for the system to classify are now automatically queued for human review and potential retraining.

### UnknownBench-v3 — 177 novel attack prompts, 3rd-generation strategy

Four new benchmark datasets (4 × ~45 prompts) using structurally different generation strategies from both v1 and v2:

### Multilingual Tier 2.5 — Romanised script detection

Added language detection for all-Latin text to close the Romanised injection gap. When a prompt is all-Latin but `langdetect` identifies it as non-English (Arabizi, code-switched text, etc.), it is translated to English and re-checked against the Tier 2 phrase patterns.

Limitation: Pinyin (Romanised Mandarin) remains the hardest case — `langdetect` cannot distinguish Pinyin from random Latin syllables. This is documented.

## What FIE is — and what it is not

FIE is a **failure intelligence and diagnosis system**. It identifies why an LLM interaction is likely to fail, surfaces evidence, provides calibrated confidence estimates, and learns from human feedback.

The detection pipeline is one part of the system. The larger objective is to help developers understand adversarial risk, hallucination risk, and interaction-level failures before they reach production.

**FIE is not a general content moderation system.** It will not reliably detect hate speech, harassment, or self-harm requests that are phrased as normal conversational requests without adversarial structure. The evaluation numbers below reflect this scope constraint.

No configuration, no API key, no network calls. Everything runs locally with bundled models.

## Evaluation Results

Evaluated against 2,006 prompts across 8 public benchmark datasets (AdvBench, JailbreakBench, Anthropic Red Team, HarmBench, OpenAI Moderation Evaluation) with 1,521 labeled attack prompts and 485 benign prompts.

**All results use only the local 11-layer pipeline. No Groq, no LlamaGuard, no external API calls.**

Phase 2 stress-tests FIE against attacks specifically designed to evade it. All experiments use frozen benchmark datasets with SHA-256 integrity manifests.

### Exp H1 & H2 — TruthfulQA evaluation (817 questions, 38 categories)

Harness at `evaluation/hallucination/run_eval.py`. Compares full FSV + XGBoost (H1) against ensemble disagreement alone (H2).

Labeling: substring containment check on correct answers → ROUGE-1 recall fallback. Ground truth from TruthfulQA `correct_answers` field only.

Shadow ensemble: `llama-3.3-70b-versatile` (primary) + `deepseek-r1-distill-llama-70b` + `qwen-qwq-32b`.

**The 560-feature FSV is dominated by 10 features.** The remaining 550 features add no signal. This is the hallucination-side equivalent of the adversarial Exp 1 vs Exp 2 finding: complexity ≠ signal.

## Known limitations

**What FIE misses:**

- **Content moderation** — hate speech, self-harm, sexual content phrased as normal requests (54% recall). Not the design target.
- **Romanised Pinyin** — `langdetect` cannot reliably identify Pinyin as Mandarin Chinese. Still caught by PAIR at semantic level in many cases.
- **Subtle crescendo** — when the final harmful turn uses indirect language, trajectory boost alone is insufficient (56% overall recall).
- **White-box evasion** — an attacker who reads this codebase can craft prompts below threshold. FIE is not designed to be a black box.

**What FIE does well:**

- **Near-zero false alarm rate** — 97.5% precision means minimal disruption to legitimate use.
- **GCG suffix detection** — 96% recall on gradient-optimized adversarial suffixes.
- **Novel unknown attack detection** — 97.18% TPR on prompts the system has never seen (PAIR v4, UnknownBench-v3).
- **Self-correcting via feedback** — confirmed attacks go into a fast-block set immediately; UNCERTAIN-zone true positives are automatically staged for retraining.

## How detection works

FIE runs **11 detection layers in parallel** using a `ThreadPoolExecutor` (10-second hard timeout). Results are aggregated by weighted voting through a three-zone classifier:
