Page shell for every report. The report is a single self-contained file: the
CSS and JS are inlined (passed in as the safe `css` / `js` context values, read
from styles.css and report.js), so the document renders correctly opened from
disk with no network access and survives being emailed as one attachment.
Child templates (report.html, timeline.html) fill the `body` block.
