Every recorded check against its specification limits. A point outside the band is a failure, and each one opened the non-conformance listed below — the link is the audit trail, not an inference.

Raised automatically when a measurement falls outside spec, and then worked: taken under review, decided about (use as is, rework, scrap, return) and only then closed. Each step is a supervisor action recorded against the person who took it.
