 IMPRESSION: 
 
 Left subclavian line ends at the origin of the SVC.
 Tracheostomy tube midline.
 Consolidation at the base the left lung is long-standing, probably atelectasis.
 Large pneumoperitoneum persists, suggesting extravasation related to gastrostomy placement or gastrointestinal perforation perforation.
 Mild pulmonary edema and moderate left pleural effusion and moderate to severe cardiomegaly have all worsened.
